Completing the Newly Hired Animal Handler or Related Work - Intake form and Animal-Related Medical and Allergy Surveillance Form (initial and ongoing)
Questionnaires
When the above MyChart is created and completed, you can access the forms.
Login to MyChart, select: Menu, Questionnaires (there are three)
-Only persons new to UK must complete the Newly Hired Animal Handler or Related Work - Intake Form
and
-Animal-Related Medical and Allergy Surveillance Form (initial and ongoing)
Complete both forms entirely. Click submit. The forms will be reviewed by an Occupational Health Registered Nurse (RN) in University Health Service (UHS).
Later, after regular work, only those needing to report a change in species to UHS must complete:
-Change in Animal Handler Species – Protection Re-evaluation Form,
and
-Update your Animal-Related Medical and Allergy Surveillance Form (initial and ongoing).

Approval/not approved status, or additional educational animal handler information will be determined, and a letter will be sent to you via your MyChart (see Menu, Letters). Check Letters to view the status of your Surveillance Form. You should receive a text or email informing you of new information.
